Encrypted Form Instructions
How to fill the form correctly, what to watch for, and exactly what to do after you encrypt and submit
Already paid but closed the browser?
You do not need to pay again. Open Resume Form, enter the resume passphrase you were given after payment, then unlock the form with the separate access passphrase.
The full path (in order)
- Pay the $30 application fee if you have not already.
- Unlock the secure form with the access passphrase (case-sensitive).
- Fill every required field (*) — use the Section progress bar as a guide.
- Encrypt & submit, then wait for the success message.
- Press Clear All before you leave the page.
- Send the encryption passphrase separately (call or separate text — not with anything else).
Before You Start
- Keep your internet connection active the entire time. The encrypted form is delivered automatically after you submit.
- You will need the access passphrase that was given to you separately (default is usually FAC-SECURE unless you were told otherwise).
- Have your personal information ready: full legal name, date of birth, current address, phone, email, last four of SSN, any conviction/release dates, previous addresses, and character references.
- The $30 application fee must already be paid before you complete this form.
- Encryption uses strong multi-layer AES protection and may take longer on some phones — this is normal.
Why security is held to a high standard
The form uses multi-layer encryption because it contains sensitive personal information. The encryption passphrase is sent through a separate channel and is never included with the form file itself. These are practical measures designed to protect the information you provide.
How to Properly Fill Out the Form
Take your time. The form validates as you go and will highlight problems in red.
- A sticky Section X of 6 progress bar at the top of the form shows where you are (Personal, Disclosure, Residential History, References, Declaration, Fee Status).
- Required fields are marked with an asterisk (*). Every one of these must be completed.
- Invalid or empty required fields show a red underline and a short red error bar under the field (for example: “Enter a valid email address”).
- Yellow notices on the form remind you: do not close the tab until you see the success message and press Clear All.
- Some fields automatically convert to block capitals (full name, address, employer, signature, reference names). Type normally — the form will uppercase them for you.
- Last four digits of SSN: enter exactly four numbers. No letters, spaces, or extra characters.
- Email address must be in a normal valid format (example: [email protected]).
- Character References: fill in as many of the five as you can. Include full name, relationship, contact info, and years known.
- Check the box that confirms you understand the $30 fee will be charged only after this form is successfully completed and submitted. The form will not encrypt without this check.
- Signature field: type your full legal name exactly as it appears on the first field.
Two different passphrases
Do not mix these up. They are used at different times and for different purposes.
1. Access passphrase (unlock)
Opens the form so you can fill it. Given to you separately (often FAC-SECURE). Case-sensitive. This is not the passphrase you create at submit.
2. Encryption passphrase (submit)
You choose this when you click Encrypt & Submit (min 8 characters). Write it down immediately. Send it later by a separate call or text — never with other details.
What To Do — Complete Step by Step
- Make sure you have a stable internet connection.
- Enter the access passphrase and click Unlock.
- Fill every section carefully. Use the Section progress bar. Watch for red underlines and red error bars under fields — those mean something is missing or incorrect.
- Complete the Character References section with as many solid references as possible.
- Check the box acknowledging that the $30 application fee will be charged only after this form is successfully completed and submitted.
- Click Encrypt & Submit Securely.
- When prompted, choose a strong encryption passphrase (at least 8 characters). Write it down immediately on paper or in a secure note. You will need this exact passphrase later.
- The form will encrypt with 56 layers of protection and then deliver the package automatically. You will see a success message when it is complete. This may take longer on some phones.
After Encryption & Submit — Critical Steps
Do these steps in order.
- Write down your encryption passphrase and store it somewhere safe and private. Do not rely on memory alone.
- You should see a success message confirming the encrypted package was delivered.
- Click the Clear All button. Confirm when asked. This erases all the information you typed so it is no longer sitting in the browser.
- Close the form or return to the main site.
Sending the Passphrase
- The encrypted form has already been delivered automatically.
- Give the encryption passphrase through a completely different channel — preferably a phone call or a separate text message.
- Never put the passphrase in an email or in the same message as any other sensitive information.
- Keep your copy of the passphrase until you receive confirmation that the package was received and opened successfully.
Do
- Keep your internet connection active the entire time.
- Fill every required field (*) completely and accurately.
- Double-check names, dates, addresses, and the last four of the SSN.
- Write down the encryption passphrase the moment you create it.
- Press Clear All after successful submission.
- Send the passphrase by a different method (call or separate text).
Do Not
- Do not disconnect from the internet while using the form.
- Do not send the encryption passphrase in the same message as any other information.
- Do not leave the filled form open on a shared or public computer.
- Do not skip the Clear All button after submitting.
- Do not share screenshots or photos of the filled form.
- Do not pay the $30 fee again — it should already have been paid before you reached this form.
If the Form Seems Locked or Stuck
Look for a red underline on the field and a red error message bar underneath it. These mark the problem areas.
- Scroll the entire form and fix every field that shows a red error bar.
- Confirm every required field (*) has content.
- Last four of SSN must be exactly four digits — nothing else.
- Make sure the $30 fee acknowledgment checkbox is checked.
- Email address must look like a normal email (contains @ and a domain).
- If the form will not unlock at all, double-check that you typed the access passphrase correctly (it is case-sensitive).
Quick Fix Checklist
- Look for red error bars under fields → correct those first.
- Check the fee acknowledgment box.
- Confirm the access passphrase if the lock screen will not open.
- After a successful submission, always press Clear All before leaving the page.
- If something still fails, close the browser tab completely, reopen the original form file, and start again.
